New Client Information 

As a new client, you will be required to review and complete the following intake forms prior to attending your first session: Contact Information & DemographicsInformed Consent for Services AgreementTelehealth Informed Consent for Services AgreementNotice of Privacy Policies, and Good Faith Estimate. These forms contain important information. Please review them carefully and contact us directly if you have any questions or concerns.

The forms mentioned above can be completed electronically or in person. 

To fill out the forms electronically, please contact our office by email at [email protected] or submit an Appointment Request Form (please see the field below). You will be required to have an email address in order to submit the forms. We take your privacy seriously and make every effort to ensure confidential information remains protected through the use of data security technology. An encrypted email will be sent to you with the cited documents attached and instructions to successfully complete the process.  

To fill out the forms in person, we ask that you arrive to your first session 15-20 minutes early to give sufficient time for the necessary paperwork. Intake forms are available in our waiting room and are located near the reception window. Please be aware that you must complete all intake forms before starting your first session. Existing Client Information

Existing clients can complete forms electronically or in person on an as needed basis. 

To fill out forms electronically, please contact our office by email at [email protected]. You will be required to have an email address in order to submit forms. We take your privacy seriously and make every effort to ensure confidential information remains protected through the use of data security technology. An encrypted email will be sent to you with instructions and the requested forms.

To fill out forms in person, please directly refer to your psychotherapist for assistance at your next scheduled session.
